Thanks a lot for the log! After first review we couldn't find anything relevant to the issue in the log file. Going to do a second review to make sure we're not overlooking something important.
For your information, we are adding more diagnostics to the add-on, creating a temporary version to debug this specific issue. Unfortunately we're unable to reproduce it locally so far and need your help with troubleshooting.

Please verify C:\Users\your user name\AppData\Local\Microsoft\SQL Server Management Studio\19.0_IsoShell\Extensions\SoftTree Technologies\SqlAssist.vs\1.0\extension.vsixmanifest file. You can open this file in any text editor. Please check it contains the following line
<MefComponent>C:\Program Files (x86)\SQL Assistant 12\bin\SqlAssist.vs.14.dll</MefComponent>
If it's not there, or the path is incorrect, the installed file is bad and needs to be updated. You can copy it from SSMS 18 version, they should be identical C:\Users\your user name\AppData\Local\Microsoft\SQL Server Management Studio\18.0_IsoShell\Extensions\SoftTree Technologies\SqlAssist.vs\1.0\extension.vsixmanifest
In addition, the following cache files will need to be deleted before SSMS restart
C:\Users\your user name\AppData\Local\Microsoft\SQL Server Management Studio\19.0_IsoShell\Extensions\*.cache files will need to be deleted, only files with .cache extension
C:\Users\your user name\AppData\Local\Microsoft\SQL Server Management Studio\19.0_IsoShell\ComponentModelCache\*, all files in this folder

Compared both files "extension.vsixmanifest" from 18 and 19 folders and they are absolutely identical.
The path to the dll exists and the file SqlAssist.vs.14.dll is in the right folder and has the right version 12.3.427.
Deleted cache files from folders, you wrote and started the studio. Still the same.

Would you mind temporarily renaming C:\Program Files (x86)\SQL Assistant 12\bin\SqlAssist.Sms.dll to .bak or something? Please don't restart SQL Assistant if it's running. Rename it while SSMS is not running, and then clear cache files again and start SSMS. What do you get after that?

Many thanks for your persistence and help. With your help we managed to reproduce the issue. It appears that a combination of some Visual Studio versions and SSMS 19.1 lead to incorrect platform version detection causing SQL Assistant to activate wrong add-on code. We are working on a solution for this issue.

We have emailed download link for version 12.3.431 to all users who responded to this topic. This version is compatible with the latest SSMS 19.1.56. If you are impacted too, but not participated in troubleshooting and would like to try version 12.3.431, please email to support for further assistance.
