This support board sanitizes messages and removes part of text it considers potentially unsafe, html/xml tags are sort of disallowed.
Can you please confirm that in your extension.vsixmanifest, the content of the Identifier line matches the below line and not what appears like missing in the sanitized version above?
Identifier Id="SqlAssist.vs..3d117742-b761-4b83-86ce-ca2aa58a8660"

One more thing. Do you have by any chance a second copy of extension.vsixmanifest in
C:\Program Files (x86)\Microsoft SQL Server\160\Tools\Binn\ManagementStudio\Extensions\SoftTree Technologies\SqlAssist.vs\1.0\extension.vsixmanifest
If yes, does it have the same content and the same file path referencing C:\Program Files (x86)\SQL Assistant 12\bin\SqlAssist.vs.14.dll?

Here is what we think may resolve this issue.
Please try deleting C:\Users\your user name\AppData\Local\Microsoft\SQL Server Management Studio\19.0_IsoShell\ComponentModelCache. This needs to be done while SSMS isn't running. It will rebuild that folder with cached files on startup.

Sorry couldn't reply soon enough. Here's what I found:
*No line with Identifier Id="SqlAssist.vs..3d117742-b761-4b83-86ce-ca2aa58a8660" in SADiag output.
*Deleting ComponentModelCache didn't fix it
*No extra vsixmanifest. In fact, that folder does not exist at all.

I got it working. In addition to deleting the "ComponentModelCache" folder, I deleted "extensions.*.cache" in %localappdata%\Microsoft\SQL Server Management Studio\19.0_IsoShell\Extensions as well.

First of all, congrats for getting it to work!
This is very interesting, we will look into that, I don't think extensions.*.cache files are referenced in any documentation. Usually deleting ComponentModelCache is sufficient.

Thanks. I renamed ComponentModelCache and extensions.*.cache so I have the ability to restore them if something went wrong. Tried just renaming ComponentModelCache and it definitely doesn't fix the problem. I needed to do both.
