Hello Team, I just did a clean install and have the same issue. SSMS v18.10, SQL Assistant 12.0.191.
Ctrl + Shift + F5 shows exactly what people here described. Warning about the unsupported window class ( ), None in the fields and disabled OK button. I can change everything except the Target Window Class and the OK button stays disabled.
Any idea how I can resolve this issue?
EDIT: noticed that my username is email. Is it possible to change it to something or delete my account? Tx in advance.

This message indicates that SQL Assistant add-on for SSMS is not installed, or not registered correctly. That's why everything is disabled. Please try disabling add-on in SQL Assistant options and then re-enabling it again. Usually that does the trick.

Yes, I've tried to disable and re-enable the add-on. I've even uninstalled and then re-installed the SQL Assistant. Basically, I followed all of the suggestions in this thread. Note that this is a fresh install (i.e. there were no previous versions of SSMS or anything like that, it is a new PC).
P.S. Thank you for changing the username!

Could you please launch the self-diagnostics test from the About tab or command line and share the results?

Sure, please check the attached. Btw, just to confirm, I did search for the ComponentModelCache folder, found it in VS and SSMS. Deleted both. Restarted PC. Started SSMS and it didn't help. After that, I've created this diag file.

Thank you. The log is showing that SQL Assistant's add-on for SSMS 2019 has been registered but is not getting loaded. This might be caused by security settings. Please try the following.
Close SSMS, and close SQL Assistant system tray application.
Start SQL Assistant in "Run as Administrator" mode. Right-click the SQL Assistant icon in the system tray and choose Options in the context menu to open the main Options dialog.
On the Targets tab, select SQL Server Management Studio, and then on the right expand the Advanced section.
Set the Register SQL Assistant Add-on option to No, and click the Apply button.
Now set the Register SQL Assistant Add-on option to Yes, and click the Apply button again.
Start SSMS in "Run as Administrator" mode, open new query window and verify SQL Assistant add-on is active in that window, you can simply right-click to see if the menu is there, or just start typing SELECT..
If that works, restart everything normally not using Run as Administrator mode and test SSMS again. If it works, you are done. If it doesn't, then it very likely the issue is caused by Windows User Account Control reverting some add-on registration or installation changes

I suspect there is some issue caused by conflicting setup for SSMS and VS, and yet I'm not able to pinpoint from here the exact place and reason. You would need to open a support case to troubleshoot it further. Or alternatively you can wait a bit and try version 12.1 expected by next Monday and see if it works for you. I know there are some internal updates in 12.1 containing code compatibility changes for latest Microsoft releases.

Hello!
Unfortunately, I still have exactly the same issue after 12.1 release.. updated using the update button in the about dialog-> didn't work. Started as admin, unregistered, registered integration -> still doesn't work.
Any suggestions?
Tx!
Btw, I've noticed VS 2017 mentioned in the diag file. I don't have it installed, just FYI.
Here is another diag file (using admin access):
