If you are checking DB Explorer, it has its own refresh independent from the cache. The content of DB Explorer is database driven rather than cache driven. Add a new table in a database, refresh cache and then try a SELECT * FROM and see if you have that table in popup. I don't need to refresh the cache manually, it's done automatically in the background for when SQL Assistant senses DDL operations.
