I see, this is a system where SQL Assistant has been previously installed and has old add-on registration entries for SSMS 2008.

Here is what you need to do. Start SQL Assistant in "Run as Administrator" mode. Open SQL Assistant's Options dialog. On the target tab, select "MS Management Studio", on the right, expand 'Advanced" section, set "Register SQL Assistant Add-on" to No and click Apply, then set it back to Yes, and click Apply again.

Now start SSMS 2012 in "Run as Administrator" mode and on start up it should load the add-on and complete the installation. Please let us know if that helps.
